HOW TO SEE YOUR DOCTOR
Doctors will see patients on a walk-in basis, depending on availability. Please note that priority will be given to patients needing urgent medical attention. To book a consultation,
call (02) 9621 1411 and talk to one of our friendly receptionists about organizing an appointment. All General Practice consultations are BULK BILLED to patients covered by Medicare. Please present your Medicare card and concession card to reception staff during your visit to be eligible for BULK BILLING. If no valid Medicare card is presented, you will be charged a consultation fee payable at the time of consultation.

After Hours Care
If medical attention is required outside our opening hours, please contact Sydney Medical Service After Hours GP on (02) 8724 6300 This is a bulk billing service. In the event of an emergency please call 000, or go to your nearest hospital.
Home Visits
Home visits are available both within and outside of normal opening hours. Please discuss with staff or your GP if you meet the eligibility criteria.
Receiving and Returning Telephone Calls
Telephone calls requiring urgent medical advice or medical emergencies will be transferred to the GP. All other incoming telephone calls from patients are recorded and will be returned by the GP during or at the end of the day.
Parking and Disabled Access
Ample car parking is available nearby. Disabled parking is available at the front and rear entrances of Hope Medical Practice (Town Centre Building) on Campbell Street and Colo Lane, respectively.
Recalls/ Reminders for Follow Ups
Doctors at Hope Medical Practice may send you reminders from time to time regarding preventative health care issues and services offered (e.g. pap smears, immunisations).
If you do not wish to be enrolled in this reminder system please advise the reception staff or your doctor. It is the patient’s responsibility to return to the doctor to discuss the test results. To maintain privacy and confidentiality our reception staff will NOT give test results over the phone. Our reception staff or Doctor will contact you for recall on clinically significant tests results including pathology and diagnostic imaging.
Medical Records and Privacy
Medical records will be kept of your visits and treatment. Your medical records are confidential. The contents will be divulged only with your consent or where justified by law. You are entitled to see or correct your patient records at any stage. Hope Medical Practice Blacktown complies with the Privacy Acts, including the way we collect, store, use and disclose health information.
Patient Feedback
From time to time, Hope Medical Practice invites patients to complete a questionnaire on their views as to ways in which then Medical Practice can be improved. This survey is kept confidential and helps us to improve our services to you. We take all your concerns, suggestions and complaints seriously.
